Client Disclosure and Informed Consent

Description of Coaching: Coaching is partnership (defined as an alliance, not a legal business partnership) between the Coach and the Client in a thought-provoking and creative process that inspires the client to maximize personal and professional potential. It is designed to facilitate the creation/development of personal, professional or wellness goals and to develop and carry out a strategy/plan for achieving those goals.

During your sessions we will use one or more of the following techniques: 

1. Coaching: Your practitioner is certified as a Master Awaken Your Life Coach and is governed by the Code of Ethics set by the ICF.

2. Yoga: Your practitioner is a Registered 200-Hour Yoga Teacher (RYT-200) as accredited by the Yoga Alliance and is governed by the Code of Conduct set by the Yoga Alliance.

3. Health Coaching: Your practitioner is a Certified Health Coach through The Institute of Integrative Nutrition.

The Process 

We begin with you telling me the problem. I will ask you for examples of the problem and other questions to discover HOW you do it in your head. I will also probe to discover the source of the problem – the precipitating event(s). Some time before the end of this part of the session I will ask you what has to happen so you will know the problem is gone.

Coach-Client Relationship 

A. Coach agrees to maintain the ethics and standards of behavior established by the International Coach Federation. It is recommended that the Client review the ICF Code of Ethics and the applicable standards of behavior. 

B. Client is solely responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions and results arising out of or resulting from the coaching relationship and his/her coaching calls and interactions with the Coach. As such, the Client agrees that the Coach is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by the Coach. Client understands coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease. 

C. Client acknowledges that coaching is a comprehensive process that may involve different areas of his or her life, including work, finances, health, relationships, education and recreation. The Client agrees that deciding how to handle these issues, incorporate coaching principles into those areas and implementing choices is exclusively the Client’s responsibility. 

D. This is Alternative or Complementary Health Care: Your coach is a legal Complementary Healthcare Provider, and not a Medical Doctor, psychiatrist, psychologist. The services you receive are not licensed in this state, nor or they regulated by a governmental body. The self-regulated holistic treatments and client centered disciplines in which your practitioner is trained and in which she/he has experience include consulting and coaching, yoga, nutritional therapy and bodywork. I will always provide only those services in which I have been trained, and if I find that I cannot help you, I will refer you to a licensed person who can assist you.

Client acknowledges that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. If Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the coaching relationship agreed upon by the Client and the Coach. 

E. The Client understands that in order to enhance the coaching relationship, the Client agrees to communicate honestly, be open to feedback and assistance and to create the time and energy to participate fully in the program. 

Services 

The parties agree to engage in the Coaching Program through telephone meetings. Coach will be available to Client by e-mail in between scheduled meetings. Coach will respond to emails within 48 hours. 

Schedule and Fees 

Client agrees to pay the total amount due in the specified time period. The calls shall be 75 minutes. If rates change before this agreement has been signed and dated, the prevailing rates will apply. 

Your Guarantee 

All work that I do is guaranteed. That means that you will receive the support and follow up to ensure that you get the results wanted. 

The guarantee works like this: 

  1. You must finish the program. That means, attend every session. You must go through the entire process of the work all the way to the end.

  2. You must do the tasking after each session, and you need to do it fully and completely.

Confidentiality 

I keep all information we receive from you, confidential, and do not disclose it to persons outside the company. All client information is kept strict confidential and is for  internal use only. (Importantly, I am required in some states to share information about child abuse with state bodies who oversee such matters.) 

Email Confidentiality Policy: Email messages, including any attachments, are for the sole use of the intended recipient(s) and may contain confidential and privileged information. Any unauthorized review, use, disclosure or distribution is prohibited

Scheduling Policy 

The time of the coaching meetings will be determined by Coach and Client based on a mutually agreed upon time. The Client will call the Coach at specified number at the time of appointment. 

Appointment Cancellation Policy

The client is responsible for attending their sessions as scheduled. Should you have a schedule conflict you must cancel and/or reschedule no later than 24 hours prior to your session. Failing to do so will result in a "no show" and will count as a completed session. Please complete all sessions within your agreement time frame, any remaining sessions will expire and not roll over.

Termination Either the Client or the Coach may terminate this Agreement at any time with two weeks written notice. Client agrees to compensate the Coach for all coaching services rendered through and including the effective date of termination of the coaching relationship. 

Limited Liability

While, during the sessions, I will offer you advice and other ways of looking at any and all presenting problems and its solutions for your consideration. You hereby agree that what we discuss is only our perspective and is not binding upon you, nor is it prescription. If you want to discuss my suggestions with someone else, you should discuss them with a licensed health care provider. It is your responsibility to confirm whether or not any changes we made produced the desired results. It is your responsibility to communicate your results to me. My liability is limited to the amount paid for the work.
